Dear Parent/Guardian:

Thank you for your interest regarding registering your child in the Amherst Central Schools. If you are a parent resident of the district, completing this registration form will begin the registration process. If you are unsure whether your residence falls within the district boundaries, the attached district map should help you. In addition, the district web site, each school and the District Attendance Office have a listing of streets within the district so that you can check the location of your residence. Any adult registering a student will be required to present photo identification for security reasons. Please fill out the attached registration form for each student you are registering, and sign the declaration and authorization on the reverse side of the form. If you complete this form at home, a notarized signature is required.

As mandated by New York State law, all families registering children in Amherst Central Schools must produce two proofs of residency within the district. Depending upon your family’s situation, Federal and State laws also require school officials to obtain proof of citizenship, visa or other proof of immigration status, date of entry into the United States, current custody/visitation orders, guardianship order, deed, lease, notarized letter, affidavits or other documentation in order for registration to proceed.

A school administrator will review your completed registration form and proofs of residency. School personnel will confirm residency status by telephone within 24 hours of receiving all required documentation. You will then be able to schedule an appointment at school for testing, student schedule review and first day of attendance. Until this process is complete, your child is required to attend his/her current school.
