Sport Signups
Amherst athletics is pleased to notify athletes and families that we will be using a new sports registration platform called FinalForms. Fall sports signups will open approximately 30 days prior to the start of the season (end of July). All signups must be entered via FinalForms. See links below for more information and to sign up!

Sports Physicals
To participate in sports, a current physical and immunization record must be on file with your child's health office. The exam must be dated within one year from the start date of the selected sport (ex. fall sports - 8/1/21 or later). Physicals can be uploaded to the FinalForms registration platform or delivered to the school health office (info below):

Athletes that have previously tested postive for COVID-19 must provide a medical release note from their primary care physician. Your PCP may require a return to play protocol (RTP), this process can be completed in coordination with our Athletic Trainer. For more information regarding RTP after positive COVID-19 diagnosis, visit the American Academy of Pediatrics website.
