Students interested in playing a winter sport need to complete the following Google form by January 1st (link will be active starting on 10/30):
Parents/Guardians are required to complete athletic consent forms via Aspen. The parent portal will open 30 days prior to the start of the fall season. For instructions on how to complete parent/guardian athletic consent forms in Aspen, click the link below.
Sports Physicals
All student-athletes must have a health/physical examination on file with their school health office (Middle School or High School). Amherst CSD does accept and recommend health/physical examinations to be completed by your child's Primary Care Provider (PCP). Health/physical examinations must be dated January 2020 or later to be valid for the 2020-2021 winter sports season. On December 17, 2020 we received a memo from the NYSED that allows physical exams dated for the 18-19 & 19-20 school year to be accepted for any sport season during the 2020-2021 school year. Note, for physicals that are dated more than 30 days prior to the start of the winter season, an updated Interval Health History form must be submitted to the school nurse prior to participation.
